This document provides answers to frequently asked questions about the National Registry Driver Examination Forms. <br /><br />Medical examiners (MEs) are required to use the newly revised driver examination forms starting from December 22, 2015. The forms can be found on the National Registry and FMCSA websites, and can be completed electronically or by hand. The forms are not available for purchase. <br /><br />The newly revised driver examination forms must be used as is, with only a few minor changes allowed. These changes include adding a bar code, resizing the instructions, making the instructions available separately, and filing/storing the form without the instructions. The forms cannot be modified in any other way. <br /><br />The commercial motor vehicle (CMV) driver examination forms have been updated with new qualification categories and additional driver information. The qualification selection on the forms depends on whether the driver operates in interstate or intrastate commerce. <br /><br />MEs are required to ask drivers about their qualification needs and to be aware of waivers and standards for intrastate drivers. MEs should always make qualification determinations according to the relevant standards. <br /><br />MEs may issue a second opinion or request additional information before making a qualification determination. Drivers are responsible for disclosing accurate and complete medical history. MEs should complete the CMV Driver Medical Examination Results Form for each examination, and submit it to the National Registry. <br /><br />The Medical Examiner's Certificate (MEC) Form must be completed and issued to qualified drivers. MEs have discretion in determining the duration of the MEC, with 3-month MECs allowed. MEs are required to conduct a complete examination each time a qualification decision is made. MEs must provide drivers with the original paper MEC. <br /><br />Definitions of terms such as commercial learner's permit (CLP), commercial driver's license (CDL), and interstate and intrastate commerce are provided.
