Portable sleep monitoring devices are being considered as an alternative to polysomnography (PSG) for diagnosing obstructive sleep apnea (OSA). However, the accuracy and reliability of these devices compared to PSG is uncertain. A literature review found that portable monitors performed reasonably well compared to PSG in diagnosing OSA, but no device had perfect sensitivity and specificity. Level 3 monitors showed the strongest evidence for diagnosing OSA. Manual scoring of portable monitors provided more consistent results with PSG. Randomized controlled trials also supported the utility of portable monitors in diagnosing OSA. However, further research is needed to confirm the utility of Level 2 and 4 devices. Portable monitors offer a convenient and cost-effective alternative to PSG for diagnosing OSA, but their diagnostic performance should be interpreted with caution.
