The Federal Motor Carrier Safety Administration (FMCSA) requires all initial and renewal requests for Skill Performance Evaluation (SPE) certification to adhere to certain regulatory requirements. The application must include the following information: the names and addresses of the motor carrier co-applicant and the driver applicant, the driver's DOT Motor Carrier Identification Number (if known), and a description of the driver's limb impairment for which the SPE certificate is requested. <br /><br />The type of operation the driver will be employed to perform must also be described, including the states in which the driver will operate, the average period of time the driver will be driving and/or on duty per day, the type of commodities or cargo to be transported, and the type of driver operation (e.g. sleeper team, relay, owner operator). The application should also include the number of years of experience operating the type of commercial motor vehicle requested, as well as any modifications made to the vehicle for the driver applicant.<br /><br />The application must be accompanied by a copy of the results of the driver's medical examination report, the medical certificate, and a medical evaluation summary completed by a board-qualified or board-certified physiatrist or orthopedic surgeon. The medical evaluation summary should assess the driver's physical capabilities as they relate to the ability to perform tasks associated with operating a commercial motor vehicle, and for amputees, the examiner must assess the driver's ability to demonstrate precision prehension and power grasp prehension with each upper limb separately. <br /><br />A copy of the driver's road test, or a copy of both sides of the commercial driver's license, must also be included, as well as a copy of the driver applicant's application for employment and any previous SPE certificates or waivers of certain physical defects issued by the individual state(s). Finally, a copy of the driver applicant's state motor vehicle driving record for the past three years from each state where a driver's license or permit has been obtained should be included.<br /><br />Once the application is completed and submitted, the Division Administrator/State Director of the FMCSA will review and either grant or deny the application for SPE certification. If granted, the terms, conditions, and limitations of the SPE certificate will be specified. The certificate is valid for up to two years and may be renewed prior to the expiration date. Falsifying information in the application is prohibited. A sample initial application and additional instructions are provided for reference.
