This report describes a CDC/ATSDR Assessment of Chemical Exposures (ACE) conducted after the February 3, 2023 train derailment and chemical fire in East Palestine, Ohio, near the Pennsylvania border, affecting nearby communities including Darlington Township, Pennsylvania. Chemicals of concern included vinyl chloride and other industrial compounds; evacuation and shelter-in-place orders were issued, and a controlled “vent and burn” of vinyl chloride railcars occurred on February 6, with evacuation orders lifted by February 8.<br /><br />From February 21 to March 31, 2023, investigators collected cross-sectional community survey data (N=701) via temporary Health and Resource Centers (65%), self-administered online surveys (28%), and door-to-door outreach (7%). Participants were mostly White (96%), non-Hispanic (94%), and female (63%), with a median age of 57.<br /><br />Most respondents reported potential hazardous chemical exposure (86%), primarily through air (96% of exposed), and many reported contact with smoke (63%) and dust (47%); 83% noticed odors, most commonly “chemical.” Health impacts were frequently reported: 92% had at least one new or worsening physical symptom, commonly ear/nose/throat symptoms (80%) and headache (76%), along with coughing and eye irritation. Mental health symptoms were also common (70%), especially anxiety, fatigue, sleep difficulties, irritability, and nervousness.<br /><br />Less than half of symptomatic participants sought medical care (41%), typically 6+ days after the incident, and 44% reported unmet needs—most often water, plus medical and mental health care. Logistic regression suggested adults aged 18–64 had higher odds of ophthalmic, neurological, dermatological, and mental health symptoms compared with those 65+, and symptom reporting varied by survey method (higher at health centers).<br /><br />Findings supported establishing a permanent health clinic and ongoing health monitoring, while noting limitations such as self-reporting, convenience sampling, and lack of biological testing.
