Pdf Summary
This document provides a summary of case studies discussed in an ACOEM MRO online course. In Case Study 1a, a donor's drug test results were positive for morphine, methamphetamine, and amphetamine. The MRO should have the employer order a d/l isomer separation for methamphetamine and consult a forensic toxicologist regarding the nutritional supplements mentioned by the donor. In Case Study 1b, the correct actions for the MRO to take are ordering d/l separation for methamphetamine and consulting a forensic toxicologist. In Case Study 1c, the MRO should report the test as positive for both methamphetamine and amphetamine based on the d/l separation results. The MRO should not wait for confirmation results before reporting to the employer. In Case Study 2a, the MRO should report the test as positive for amphetamine, as none of the explanations provided by the donor are valid. In Case Study 2b, the only medication that can explain a positive amphetamine result, but negative methamphetamine, is Adderall. In Case Study 3a, the possible reasons for a negative hydrocodone result in the presence of positive hydromorphone are: hydrocodone being present but below the cutoff level, hydrocodone metabolizing more quickly than the metabolite, and the donor taking Dilaudid but submitting a hydrocodone prescription. In Case Study 3b, the MRO should report the test as negative for hydromorphone to the DER. The MRO should also consider reporting a workplace safety concern, which is covered in Module 4 of the course.
