Pdf Summary
In this document, several case studies are presented regarding drug testing results and the actions that the Medical Review Officer (MRO) should take.

In Case Study 1a, a donor has positive results for morphine, methamphetamine, and amphetamine. The MRO should order a d/l isomer separation for methamphetamine cases and consult a forensic toxicologist regarding nutritional supplements. The MRO should also note that suboxone, a medication for substance use disorders, would not cause a positive test for amphetamines.

In Case Study 1b, the MRO orders d/l separation for methamphetamine and consults a forensic toxicologist regarding supplements.

In Case Study 1c, the toxicologist confirms that the supplements do not explain the findings, and the d/l separation shows positive results for methamphetamine and amphetamine. The MRO should report these findings to the employer's Designated Employer Representative (DER).

In Case Study 2a, the donor reports various explanations such as poppyseed bagels, Inca tea, Ritalin, and CIPRO. However, none of these explanations can account for the positive amphetamine result. The MRO should report this as a positive for amphetamine.

In Case Study 2b, the MRO must identify which medication could explain a positive amphetamine result without methamphetamine. The correct answer is Adderall.

In Case Study 3a, the urine test is positive for hydromorphone but negative for hydrocodone, despite the donor having a valid prescription for hydrocodone. The possible reasons for this are that hydrocodone may be present below the cutoff, it may metabolize more quickly than hydromorphone, or the donor may have taken Dilaudid but submitted a hydrocodone prescription.

In Case Study 3b, the MRO should report the test as negative since hydrocodone is negative. The MRO should also consider whether taking hydrocodone raises a workplace safety concern.

These case studies highlight the importance of thorough analysis by the MRO and consideration of various factors in drug test results.
